3. Certificate of GWA/GPA, or equivalent

4. Colored ID photo (600x600 pixels or 300 dpi)

5. PSA Birth Certificate, if single; OR Marriage Certificate, if married (for female applicants only)

6. Certificate of English Language Proficiency such as TOEFL or equivalent (for Foreign/International applicants only)

7. A preliminary research proposal (for MS HMS applicants only ). Download the template here.

8. Recommendation from two (2) superiors and/or former professors. Fill up the form here.

9. Application fee

   • Filipino applicants: Php 500.00

   • Foreign resident: Php 500.00

   • Foreign (non-resident): USD 25.00
